面談記録やヒヤリハット報告は、書き始める前の整理だけで気力を使います。現場では「何が起きたか」は頭に残っていても、それを客観的な文章へ変えるところで時間が溶けがちです。

この回では、Gemini の `Gems` を使って、面談記録と申し送りを自分専用の型へ落とし込む方法を整理します。ゼロから書く負担を減らしつつ、利用者さんやご家族と向き合う時間を守ることが目的です。

## 記録業務で時間が溶ける場面

**結論**

面談記録、ケース記録、事故報告書は、「思い出す」「整理する」「書式へ整える」の 3 段階に時間がかかります。Gems はこのうち後半 2 つを肩代わりできるため、残業を減らしやすい領域です。

特養などの現場で記録が重くなるのは、入力作業そのものより、「読める文章に整える」工程が長いからです。メモは取れていても、主訴、対応、今後の方針へ並べ替える時点で手が止まりやすくなります。

先に切り分けたいこと

Gems は「考える責任」をなくす道具ではなく、「毎回同じ書き方へ整える手間」を減らす道具です。判断や最終確認は人が持ち、文章化だけを短縮する発想が定着しやすくなります。

## Gems を使うと何が変わるか

Vol.1 の音声入力だけでも、ゼロからキーボードで打つよりは楽になります。ただ、毎回同じ依頼文を入力するのは現場では長続きしません。

Gems を使うと、役割と出力形式をあらかじめ覚えた「自分専用の清書係」を持てます。面談記録用、SOAP 記録用のように役割を分けておくと、メモを投げるだけで必要な型へ変換しやすくなります。

Gems が向いている記録

- 出力の型が毎回ほぼ決まっている記録
- 箇条書きメモや音声入力から整える記録
- 事実、対応、結果を分けて書きたい記録

一方で、相手の感情の機微や個別事情をどう表現するかは人が最後に見直す必要があります。便利さと責任の分担を最初に決めておくのが大切です。

## 実践レシピ1 面談記録 Gems

家族面談では、会話中にすべてを文章で残そうとすると、相手の表情や間の取り方を見失いがちです。そこで、面談中は単語レベルのメモだけに絞り、終了後に Gems へ整えてもらう流れへ変えます。

Gems 作成の 3 ステップ

- Gemini の \`Gems マネージャー\` から新しい Gem を作成する
- 名前を \`家族面談まとめ\` など、役割が分かるものにする
- 出力順序と文体を指示書で固定する

そのまま使い始めやすい指示書の例です。

使う時は、面談メモをそのまま貼るだけで構いません。たとえば `長男 食欲低下を心配 とろみ食を提案 同意あり 明日開始 来週電話` のような断片でも、記録の骨組みは十分作れます。

## 実践レシピ2 SOAP 記録 Gems

申し送りやケース記録は、面談記録よりも日常的に発生する分、少しの短縮でも効果が大きくなります。施設内で SOAP 形式を使っているなら、最初からその型へ合わせた Gems を用意しておくと迷いが減ります。

運用で先に決めたいこと

個人名やセンシティブ情報の扱いは、施設のルールに合わせて統一してください。最初は匿名化した練習用のメモで試し、どこまで入力するかをチームで決めてから本運用へ移る方が安全です。

## よくある質問

### 面談記録を AI に整えさせると冷たい文章になりませんか

最初の出力は整いすぎることがありますが、文体や語尾を指示書で調整できます。重要なのは、面談の温度感を決めるのは AI ではなく、最後に見直す職員だと割り切ることです。

### SOAP 形式にしても結局手直しが必要ではありませんか

手直しは必要です。ただし、ゼロから書くのではなく、骨組みを AI が作った状態から直せるため、負担のかかり方が大きく変わります。
